STEP 2. DOCUMENTS REQUIRED:
Once Pre-Registration has been completed, parents MUST submit:
1. Birth certificate or other document of proof (NOT a copy)
AND
2. Current immunization record or exemption certificate
*These documents must be submitted within 30 days of the student's start date at Ephraim Elementary School!*
Upon enrollment of a student for the first time in a particular school, within 30 days parent/guardian must provide either a certified copy of the student's birth certificate, or other reliable proof of the student's identity and age, together with an affidavit explaining the inability to produce a copy of the birth certificate (53G-9-302).
The school office will make copies of these documents and secure them in your student's file.
STEP 3. POWERSCHOOL REGISTRATION:
This portion of the registration process is REQUIRED before the student starts school at Ephraim Elementary:
- PowerSchool log in page: https://www.ps.ssanpete.org/public/home.html
- Enter Username and Password
- Select "Registration Forms"
- Complete each form
- The office will get notified when registration is complete
STEP 4. COMPLETE MEAL APPLICATION:
The free and reduced meal application is available in your PowerSchool account. We encourage everyone to apply!

STEP 5. CLASS PLACEMENT:
Registration is complete. Thank you!
We will place your student in a class and you will be able to see what teacher they have in your PowerSchool account.
